Application Forms

Please note the vast majority of transactions on our platform are online only. A list of these online transactions can be found here.

We also offer an Upload & Send Documents function that allows you to upload documents from a list of instruction types (including some offline application forms). The documents are automatically sent to the correct team to complete your request, saving you time and reducing turnarounds times.

In the sections below, you’ll find application forms relating to tasks you cannot currently complete online.

Here you’ll find all the Investment Account forms for processes that currently cannot be conducted online.

Investment Account forms (offline processes only)

When should I use this form?

  • If you are moving money from an Investment Account held by your client to an unrelated third party's ISA.
  • If you are moving money from an Investment Account held by one spouse to the other spouse's ISA.

Please note all other Bed & ISA tasks can be completed online, including if you wish to move money from a joint investment account (held by both spouses) to an ISA for one of the spouses.

You will also need the clients owning the source Investment Account and the target ISA Account to complete and sign a Deed of Gift form which should be returned with the Bed & ISA third party form.  If you submit via ‘Upload and Send’ you should use the ‘Instruction Form’ Section for the Bed & ISA third party form and upload the Deed of Gift form as a supporting document.

When should I use this form?

  • When transferring shares from a corporate nominee account to an existing Investment Account.

When should I use this form?

  • When depositing paper share certificates in an Investment Account that has already been opened.

Stock deposit instruction form

 

When should I use this form?

  • When transferring a certificated security to us in uncertificated form.

CREST transfer form

 

When should I use this form?

  • If you wish to transfer ownership from one holder to another.

Please note if you wish to complete a stock transfer to a joint account then you will also need to complete a Joint Holder Supplement form.

When should I use this form?

  • If you wish to complete a stock transfer to a joint account then this form will need to be completed in addition to a stock transfer form.

When should I use this form?

  • When transferring holdings in an Investment Account to another individual or company.

SICAV stock transfer form

 

Who should I use this form?

  • When selling an ISA or an investment holding of a Fidelity investor who has died.

Selling in the event of the death of a Fidelity investor

 

When should I use this form?

  • To sell existing holdings to open an ISA using an inherited ISA allowance.

When should I use this form?

  • To transfer assets into an Investment Account outside of an ISA.
